Your request has been submitted to the Arkansas Department of Health, Engineering Section, Water Operator Licensing Program.   Once the request is reviewed for verification of needed fee payment and mandatory training is documented as met, the exam will be approved.  PSI Services LLC (PSI), formally AMP, will contact you to proceed with the scheduling of your license exam and payment to PSI their $69.00 facility use fee.  Additional information on the computer based exam process is at www.goamp.com, select Begin Preparing link box.  PSI should contact you within one week.

You must present to the exam session proctor a current government issued identification displaying your name, signature and picture (drivers license).
